The Clinical Significance of Light Chain Predominant Multiple Myeloma

Abstract Multiple Myeloma (MM) is a neoplastic clonal proliferation of terminally differentiated B- lymphocytes. It accounts for 1.6% all cancers and about 10-15% hematological malignancies in the United States. Monoclonal immunoglobulins provide an indication tumor burden patients with plasma cell neoplasm. Recent development has shown that proportion intact immunoglobulin producing multiple myeloma secrete much higher amounts free light chains than usual; i.e. chain predominant (LCPMM). About 18% secreting amount compared to other myelomas are associated significantly poorer clinical outcomes. In this study, retrospective investigation comparing laboratory findings conventional MM LCPMM was undertaken. Three hundred sixteen seventy-one were studied. The results remarkable marked increase death rate group group, 35.2% 13.6% respectively; P-value .00001. occurrence dialysis also markedly increased MM; 18.3% vs. 3.5% A lower eGFR observed average value 46.3 versus 69.4 ml respectively P .0000003. conclusion, confers significant disease burden, poor outcomes shorter survival. Further prospective trial will be helpful understanding pathogenesis, course, management disease. An assay serum monoclonal been described could serve as marker minimal residual such patients. We aim further study impact (LCPMM) current knowledge investigate therapeutic interventions.
